Role:
- Develop and maintain a strong rapport with clients and their families.
- Assess individual needs and create personalized care plans.
- Provide emotional support and companionship to enhance the overall well-being of clients.
- Organize and facilitate activities that promote social interaction and mental stimulation.
- Maintain accurate records of client interactions and progress.
- Prepare regular reports on the social and emotional well-being of clients.
- Communicate effectively with the care team to ensure seamless support
Requirement:
- Be a graduate of an accredited Social Work program or equivalent in a clinically relevant field.
- Be registered and in good standing with The Ontario College of Social Workers and Social Service Workers
- Proven experience in social support
- Excellent communication and interpersonal skills.
- Compassion and empathy towards the diverse needs of clients.
- Ability to work independently and as part of a collaborative team.
- Knowledge of community resources and support service
- Possess good communication, critical thinking, and organizational skills in a professional setting
- Hold a valid work permit (if applicable)
- Possess a current CPR/First Aid Certification
- Will be able to provide a clean vulnerable sector screening (VSS)
- Have up-to-date immunization records (MMR, TDAP, Varicella, Hep B, COVID-19)
- Hold a valid drivers license and have access to a vehicle (considered an asset)
